Day of Surgery
- Please leave all jewelry and valuables at home. We cannot be responsible for lost or damaged property.
- Wear comfortable clothing that is easy to put on and remove.
- We ask that you bring the following:
- Insurance cards and any necessary claim forms
- A case for your contact lenses or glasses
- A complete list of all medications (over the counter and physician prescribed), herbs, therapeutic agents, vitamins, birth control pills, patches, etc. Please
include how often you take each medication, why you are taking the medication and the last time you took each medication. You will be asked to provide this information to the clinical staff.
- A copy of your living will or advance directive, if you have one
- Arrive at least one hour prior to your scheduled time of surgery or time given to you by the pre-op nurse.
- When you arrive at the center, check in at the registration desk.
- Your surgical doctor may talk to you briefly to explain the procedure and answer any questions for you or your family.
- Your anesthesiologist will visit you in the pre-op area and answer any questions you have about anesthesia.
- For patients under 18, parents and/or legal guardian must stay in the building while the patient is in the Center
